First thing you will need…
W-9 Form: This is your tax form. Please sign and date at the bottom.
Tip: If you’re signing up as a corporation or LLC, please send in a
copy of your corporation papers along with your W-9.
Next…
A copy of your
Teaching Certificate or NYS License registration certificate (Can’t accept TEACH certificate)
Non-Teachers: A copy of your highest degree.
Copy of Liability Insurance (must renew yearly)

A copy of your
Physical with your PPD & Immunization history (must be within the last year & done yearly)
And you can use the health form that your provider gives you for your PPD.
Furthermore…
We require two professional reference letters. It can be from anyone that has known you professionally in the past two years.
You can use the reference check form that we have provided in our packet or your referral can write a letter.
Background Check: This form must be filled out thoroughly which includes addresses of where you lived in the past 25 years as well as each member of your household must be listed.

Therapist Information form:
We want information about you. We want your contact information, your modality (PT, OT, ST, SI, ABA, Service Coordinator) and what you specialize in. As much information as possible is helpful.
Criminal Background Check:
This form is strictly for criminal background checks. We do not check credit.

**All Providers including Service Coordinators will have to
complete these trainings within the first month of hire.

Service Coordinators
SC’s must complete this four part training before you can take any cases. This must be done within the first two weeks of hire and email your certificates upon completion.
All Providers:
We need proof of 10 hours of continuing education training from within the last 12 months and it needs to pertain to Early Intervention. *Any C.E. you’ve done in the last 12 months and under, send in a copy of those certificates and make sure those
certs outline how many hours of training was completed.
Early Intervention Providers:
We require a copy of your New York State Department of Health letter. **Please reach out to the DOH if you need one. (Letters are valid for 5 years)
If you do not have your DOH letter, you will be getting a W-2
NPI
If you do not currently have an NPI number, please see this link on how to obtain one. Once you have one, please email that directly to HR.

Provider Agreement:
Before any provider that is hired by Bravo Kids, Inc. can take on any cases, you must read, sign and send a copy of the provider agreement to HR.
